Ozempic®

Ozempic® is a medication used to assist with weight loss when combined with diet and exercise. Administered as a weekly subcutaneous injection, its dosage may be adjusted based on clinical response and tolerance. Carefully follow the package instructions upon receiving the product.

I agree that I:

• am 18 years or older.
• have not had a hypersensitivity reaction to Ozempic®, semaglutide, or any component of the formulation.
• have taken and tolerated the previous dosing titration (if applicable).
• do not have a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN2).
• do not have Type 1 diabetes or diabetic ketoacidosis (DKA).
• understand that the use of Ozempic® for weight loss is not approved by the FDA and constitutes off-label use.
• understand psychiatric effects, such as suicidal thoughts, have been reported with other medications for weight management.
• have not had bariatric surgery.
• understand this medication may slow gastric emptying, which can alter the absorption of other medications.
• understand this medication has been associated with acute kidney injury, diabetic retinopathy, gallbladder disease, gastrointestinal side effects, and pancreatitis.
• am not pregnant or breastfeeding.
• do not have liver (hepatic) or kidney (renal) impairment.
• understand this is not a comprehensive list of precautions, risks, side effects, or details about taking this medication.
• understand drug interactions have not been reviewed against the other medications, supplements, or herbal products I may be taking unless also filled by Bitoke.
• understand that Bitoke LLC does not provide medical care and that the providers who use Bitoke to deliver telehealth services (“Telehealth Providers”) are not a replacement for your primary healthcare provider.
• understand that the Telehealth Providers cannot:
  1. monitor ongoing health or lab values for side effects or efficacy,
  2. observe how I (or my pet) tolerate any prescribed treatment, or
  3. determine if the prescribed treatment is working as intended. I assume any risks associated with this modality of care and waive any future claims against Bitoke and/or the Telehealth Providers.
• understand that by using this site, I have established a patient-client-doctor relationship and that the Telehealth Providers are directing me to consult my primary healthcare provider soon after starting any treatment prescribed by the Telehealth Providers.
• understand that this is a supplemental mode of care for my convenience, and the Telehealth Providers are not a replacement for in-person follow-up visits or interactions with my primary healthcare provider.
